Simhachalam temple has a history of nearly a thousand years. As per the historians, the Simhachalam temple has been recognized in the 11th century CE due to an inscription on the temple by the Chalukya Chola king Kulottunga I. The temple received good attention during the period of the Chalukya Cholas. After the Chalukya Cholas, the Eastern Ganga dynasty took care of the temple. The Gajapathis came into power, after the fall of the Eastern Gangas, The Tuluva kings supported the temple up to the 16th century. After the fall of the Vijayanagara dynasty, the Muslim states of Deccan asserted independence.
In 1949, the temple came under the endowment ministry of the State Government. The family members of the Pusapati Gajapathi of the Vizianagaram state are the current trustees of the temple. They are serving the temple for the last three centuries.
